1. Use the contact form below to list your name and email as well as the name and emails of up to 8 trusted contributors who will provide you with confidential feedback. NOTE: Have your list of emails prepared before completing the form below. The form will not store partial entries.
2. Please enter your details first followed by the person you select as your Accountability Partner to support you through this program.
3. Note that for each entry you must include their name, email address and relationship to you. Do not just send us each email address. IMPORTANT: We need each name, email and relationship field to send them the proper assessment for you. Remember all this information is kept confidential.
4. Double check to ensure you have the correct email addresses for all your trusted contributors prior to completing the form below. We will use this information to send out their 360 invitations at the same time, so we need to have all their emails prior to doing that.
5. Again, you must fill each person’s relationship to you so that we can properly coordinate sending out the 360 survey.
